The area around the eyes is one of the most sensitive parts of the face and is often the first to be affected by the passage of time. Lower Blepharoplasty corrects “bags,” skin laxity, and dark circles, offering a refreshed and youthful gaze.

Dr. Drakotos specializes in innovative Transconjunctival Lower Blepharoplasty, which is performed without an external incision. This technique minimizes complications and guarantees a perfectly natural and harmonious result, improving the shape of the eyes, eliminating wrinkles, and boosting self-confidence.

THE PROCEDURE

Lower Blepharoplasty eliminates the tired appearance of the eyes and provides a youthful result, while it can also achieve an overall “lift” of the mid-face.

Dr. Drakotos specializes in the innovative Transconjunctival Lower Blepharoplasty, a safe and reliable technique with absolutely natural results. Its uniqueness lies in the ability to preserve and redistribute orbital fat. By filling the hollow areas, under-eye “bags” are removed, and dark circles and wrinkles on the lower eyelid are addressed.

This pioneering technique effectively treats the common “tear trough” deformity between the lower eyelid and the cheeks. In this way, a youthful and rejuvenated under-eye contour is achieved.

Additionally, another technique of Lower Blepharoplasty is available, performed with an incision 1–2 mm below the eyelashes. The appropriate technique is selected based on each individual case.

COMBINED PROCEDURES

It is commonly combined with procedures aimed at restoring the youthful appearance of the eyes and face, such as Upper Blepharoplasty, Facelift, and Browlift. Furthermore, it is very often combined with fat grafting, primarily in the mid-face.

HYALURONIC ACID UNDER THE EYES

Particularly in the past, but even today, under-eye concerns such as bags, wrinkles, and dark circles are often treated with hyaluronic acid injections. This practice has created a host of issues for many people and, in several cases, an aesthetically unpleasant result.

Hyaluronic acid is a tool that can be used for hydration or volume loss. However, in specific areas with a problematic response to this material, we do not recommend its use. One such area is the under-eye region.

Today, there is a solution for removing hyaluronic acid via hyaluronidase injection with Dissolve Hyaluronidase. Painlessly and effectively, the hyaluronic acid is completely removed from the area.

Following dissolution, we offer personalized restoration protocols for the lower eyelid, ranging from micro-fat grafting and PRF to the use of specialized technologies like fractional laser and fractional RF, and ultimately, permanent correction through blepharoplasty.

SURGICAL INFORMATION

RESULT
Immediate. Swelling may appear (in varying degrees), which fully subsides within 1-2 weeks.
PAIN
Minimal. Over-the-counter or prescribed pain medication is often sufficient.
STITCH REMOVAL
Stitches are removed in 5–7 days.
POST-OPERATIVE PROTECTION
None required. There are no drainage tubes.
ACTIVITIES
No hospitalization required. There are no restrictions on the patient's daily activities. Walking is encouraged. You may shower the day after surgery.
EXERCISE
Do not lift weights heavier than 10–12 kg for 2 weeks. Assuming there is no vision impairment from swelling or a requirement for pain medication, you may drive 1–2 days after surgery. Strenuous activity and heavy lifting should be avoided for at least 4 weeks. The patient can return to work within a week, depending on job requirements.
FOLLOW-UP
The patient will be monitored at the clinic during the 1st week after surgery, and subsequently at 2 weeks, 6 weeks, and 3 months, or whenever deemed necessary.

Hyaluronic acid is a tool at our disposal, but it is not ideal for all areas. Specifically, the under-eye region is one of those areas that have a problematic response to hyaluronic acid, which is why many people have faced complications. However, hyaluronic acid can be removed from under the eyes quickly and painlessly.”

ADVANTAGES OF TRANSCONJUNCTIVAL BLEPHAROPLASTY

The benefits are numerous. Firstly, the incision is internal, so there are no visible scars. We avoid muscle disruption, recovery is faster, and the probability of complications is reduced.

As Lower Blepharoplasty is a demanding procedure, excellent pre-operative planning, the surgeon’s expertise, and an individual assessment of each case are essential, as factors that could cause post-operative issues must be taken very seriously.

The main difference between modern Lower Blepharoplasty and older techniques is that fat removal is much more conservative. Instead of removing all the fat as in the past—which created a tired and "hollowed-out" look—fat is now partially removed or repositioned to adjacent areas with volume deficits.
Lower blepharoplasty is most commonly combined with facial fat grafting (especially in the mid-face) and, within the context of facial anti-aging, with a Facelift.
